Diane Pagan is a board-certified Adult Nurse Practitioner with 25 years of experience. She has held both clinical and administrative roles and has precepted over fifty APP students throughout her career. Her path has led her through a wide array of settings including nursing homes, assisted living facilities, and internal medicine offices, but she found her niche when caring for complex patients within their homes. More recently, Diane was the Director of Complex Care at West Med Group and Executive VP of Complex Care at Curis Medical Group, where she trained and managed a team of over 30 advanced practitioners who made house calls to patients throughout the five boroughs of New York.
In addition to her love of teaching others, Diane’s other passion is geriatric care. To better serve her patients, she became certified as an advanced practice registered nurse in hospice and palliative care and also as a Care Manager.
Her most recent accomplishment was fulfilling her lifelong dream of starting her own practice. Senior Moments Care, PLLC is a unique, age-friendly house call practice which merges primary care with geriatric care management. Through Senior Moments Care, Diane hopes to empower patients and caregivers to take charge of their healthcare by identifying what matters most to them.
Part of her business model is to impart as much knowledge and expertise as she can onto other nurse practitioners and students in an effort to give them an advantageous edge to take on multicomplex challenges present in the healthcare world today.
